ECU Health Beat ? Oral Cancer: Recognizing the Signs

Michael Douglas has won dozens of awards throughout his acting career, including the Life Achievement Award from the American Film Institute in 2009 and the Golden Eye for Lifetime Achievement at the Zurich Film Festival in 2010.? No doubt those award announcements were nothing compared to the announcement from his doctor that he was tumor free after an intense battle with throat cancer.? The Oscar winning star announced his throat cancer diagnosis in August, 2010, and endured an extensive 8 week bout of chemotherapy and radiation.? In January, 2011, the tumor could not be detected by physical examinations, CTs, MRIs or PET scans.? While he is not completely out of the danger zone, Mr. Douglas had a complete response to treatment and is likely on the road to a full recovery.

The occurrences of oral cancer have greatly increased over the last 20 years.? Risk factors for oral and oro-pharyngeal cancer include alcohol and tobacco use, sun exposure (lips), marijuana use and HPV infection.? The best way to avoid oral cancer?? Avoid the risk factors that can lead to oral cancer.

According to the National Cancer Institute?s website, www.cancer.gov, the leading cause of oral cancer is tobacco use in any form.? The more tobacco used, the greater the risk of developing cancer.? People who use tobacco and drink alcohol are even more likely to develop oral cancer than someone who just uses alcohol or tobacco.

However, a person who currently smokes cigarettes can lower their chances of oral cancer simply by quitting smoking.? Clinical trials have shown that within 5 years of stopping tobacco use, a person?s chance of developing oral cancer drops by 50%.? 10 years of being tobacco free lowers a person?s risk of oral cancer to that of a person who has never smoked.

Oral cancer can occur in three different regions:? the lips; the oral cavity including the front of the tongue, the gingiva (gums), the buccal mucosa (lining of the cheeks), the floor of the mouth, the palate and the area behind the wisdom teeth; the oropharynx which includes the middle part of the throat behind the mouth, the back one-third of the tongue, the soft palate (roof of the mouth), the throat and the tonsils.

A dentist, based on findings during an oral exam, usually makes a diagnosis of oral cancer.? The exam typically includes a visual exam of the whole oral cavity as well as manually feeling for abnormalities such as swollen areas or hardened masses.? Symptoms to watch for between dental visits include:

  • sores or lesions in the mouth that don?t heal
  • a sore throat or the feeling that something is caught in the throat
  • difficulty swallowing, chewing or moving the jaw or tongue
  • numbness of the tongue or other areas of the mouth
  • chronic hoarseness

If you experience these symptoms, contact your dentist immediately.

Grandmother Tests Limits In 100-Mile Swim From Cuba To Florida

HAVANA ? Marathon swimmer Penny Palfrey set out on a record bid to traverse the Straits of Florida early Friday, a 103-mile (166-kilometer) unassisted swim testing the limits of human endurance and the will of the high seas.

Slathered with sunblock and lubricant to prevent chafing, the British-born Australian bade farewell to onlookers, dove headfirst into the calm, bathwater-warm seas off Havana and began stroking slowly northward with a kayaker as escort.

"There's a lot of work that's gone into this over the past year," Palfrey said, thanking her husband, her team and Cuban officials who helped facilitate the trip. "It's all coming together, and so exciting to finally get to this point where we can get started."

After her departure, a member of Palfrey's crew was keeping fans up to date via social media.

"Swimming strong. Great conditions. Some jellies, but not bad," read one tweet. By late morning she had covered 7.5 nautical miles, or 8.6 miles, as Cuba disappeared from sight.

Palfrey will have to fight through physical and mental fatigue while fending off dehydration, hypothermia and dangerous marine life. She estimates that it will take 40 to 50 hours to make the crossing, and the currents will determine where in Florida she comes onshore.

If Palfrey succeeds, she'll go in the record books as the first woman to swim the Straits without the aid of a shark cage. Instead she's relying on equipment that surrounds her with an electrical field to deter the predators.

Australian Susie Maroney made the crossing in 1997 at age 22, but with a shark cage. American Diana Nyad made two unsuccessful cageless attempts last year on either side of her 62nd birthday, but had to call them off due to a debilitating asthma attack and painful Portuguese man o' war stings. She plans to try again this summer.

Palfrey, a 49-year-old mother and grandmother, is more than a decade younger than Nyad.

She was wearing a regular sporting swimsuit instead of a wetsuit, and planned to put on a porous, non-buoyant Lycra bodysuit that provides cover down to the wrists and ankles whenever jellyfish may be a threat.

That's particularly the case at night, according to her support team of more than a dozen navigators, handlers and medical personnel who were escorting her on the 44-foot (13-meter) catamaran Sealuver.

Palfrey is no stranger to jellyfish stings, which forced her to abort two past swims in Hawaii.

A 20-year veteran of distance swimming, her personal best came last year when she completed 67 miles (108 kilometers) between Little Cayman and Grand Cayman islands.

At a news conference Thursday, Palfrey said she swam competitively in her youth but only discovered ultra-long-distance swimming later in life.

"I spent my teenage years swimming and studying, and then I've spent a lot of time raising my family and supporting my husband's business," she said. "And now I feel that it's my window in life to do something that I love to do. I don't believe that I can do it much later in life so this is my opportunity now. "

Friday morning she was upbeat and focused on the task at hand as she prepared to set off from the Hemingway Marina in western Havana.

Asked about the first thing she planned to do on arrival in Florida, she laughed and said, "get out of the water."

"Beautiful sea, beautiful sunrise. It's a lovely morning in Cuba," Palfrey said. "Thanks. I'm gonna get started."

Lochte, Phelps advance to last showdown

(AP) ? Ryan Lochte got his busy Friday under way by qualifying fastest in the 200-meter backstroke at the U.S. Olympic swimming trials. He returned a short time later and swam in the 200 individual medley heats, advancing with Michael Phelps to their third and last showdown.

Lochte powered off the final turn in the 200 back and won his heat by more than a body length in 1 minute, 57.24 seconds. He stopped to sign autographs for a couple of young girls as he walked off the pool deck.

He came back and qualified fourth-quickest in the 200 IM, where he's the world champion, having beaten Phelps in the race at last year's world meet in Shanghai.

"The 200 back is probably one of the hardest events known to swimming," Lochte said. "It just takes all your legs out of you. Being able to come up and step up on the blocks and race like Michael Phelps, it's definitely a challenge, but I'm up for it."

Conor Dwyer, already on the Olympic team, led the way in 1:59.11. Phelps was second-fastest in 1:59.27. Austin Surhoff, the son of former major league baseball player B.J. Surhoff, was third in 1:59.65.

Also reaching the semifinals was Olympian Peter Vanderkaay in fifth.

Lochte has the afternoon to rest before returning in the evening for the semifinals of the 200 back, followed by the 200 IM semifinals.

"It's a difficult double, but it's not an impossible double," Clary said. "He's shown he's got the ability to be able to come to this meet, he doesn't really have to fully exert himself if he doesn't want to. That carries over to the other race if he's got a double that day."

In the 200 back, Tyler Clary was second-quickest at 1:57.93, followed by 100 back champion Matt Grevers in 1:58.90. Nick Thoman, runner-up in the 100 back, moved on to the evening semifinal in fifth at 1:59.24.

USA Swimming said Grevers had scratched the semifinals of the 200 back. That will allow him to focus solely on the evening 100 freestyle final, instead of having to swim the 200 back semis first.

"I do know a double would hurt me and my 100 free would suffer if I did the 200 back right before," he said before his scratch was announced. "I've got to get back by Clary and Lochte to get there (to back final). That's going to be very hard."

On Thursday night, USA Swimming wrongly announced that Grevers had dropped out of the 100 free final; the group said Friday that he had not.

"After talking to my coaches, they convinced me the 100 free is the way to go. Just cruise that," Grevers said. "My actual goal was to get a fast enough 100 freestyle time to make me a solid pick in the relay. I want to do a relay swim. The 100 free individual, there might be a shot, but I don't think I can medal in it even if I do make it."

Defending Olympic champion Rebecca Soni easily advanced in the 200 breast at 2:23.11. Micah Lawrence was second in 2:24.95, and Andrea Kropp third at 2:25.77.

"After so much sprinting with the 100, I wanted get nice feel for the 200 again," Soni said. "Just kept calm and smooth and it felt really good, really happy with that time."

Amanda Beard, trying to make her fifth Olympic team at 30, was fifth in 2:27.07.

"It's exciting, so you want to go out there and put down a good time," she said. "You don't want to use too much energy. I just wanted to feel my stroke out, have fun and see what my competition looks like."

Megan Jendrick, the 2000 Olympic champion in the 100 breast, finished 38th.

Dana Vollmer led the way in the 100 freestyle prelims, with teenager Missy Franklin also advancing.

Vollmer was timed in 54.35 seconds for the two-lap sprint. Allison Schmitt, having one of her best meets ever, was second at 54.49. Vollmer is already on the U.S. team in an individual event, while Schimitt is going for her third individual event, which would tie her with her training partner Phelps for most on the team.

Jessica Hardy, who failed to qualify for the Olympic team in the 100 breaststroke, had the third-fastest time of 54.52. Texas A&M sophomore Breeja Larson upset Soni to win the 100 breast, with Hardy third. Only the top two make the U.S. team for London.

"I never heard of her before. Ever," Hardy said about Larson. "But good for her. Now she's the gold-medal favorite."

Franklin was seventh in 54.78.

"Honestly, you have to go all out," Franklin said. "In the 100 free, there's such an incredible amount of people. It's so hard to make it back. You don't want to risk anything."

Franklin has already made her first Olympic team at 17 in the 100 back and 200 free.

"It's better than I could've asked for," she said. "I'm so excited. I'm still kind of on a high from my 100 back and 200 free. I'm just kind of riding that right now."

Natalie Coughlin, yet to secure a spot on her third Olympic team, finished ninth at 54.99. She will need to be faster in the evening semifinal to advance to the eight-woman final on Saturday.

Two-time Olympian Kara Lynn Joyce failed to make the semifinals in 18th, while 2008 Olympian Christine Magnuson tied for 22nd.
